Information & Forms for Referees

On this page you should find all the forms you require for the season along with useful information to assist you with officiating.

Forms Available:

Red Card and notes/guidance for completing a form.  Please note all forms should be completed and sent to the County Office by 9am on a Monday morning (weekend fixtures) and 9am Friday morning (midweek fixtures).   If you need any assistance in completing a form, please contact either Malcolm Jarvie (07816 416028) or Kingsley Hyland (07775 842596) as either would be happy to talk you through the form.

Match Official Abuse Form.  Completed MOA forms are required within the same timeframes as red card forms.  Again, Malcolm and Kingsley will give any assistance required if you wish to contact them.

Serious Injury Form - to be used if anyone is taken to hospital and is admitted. Please send completed forms to County Office as soon as is practicable.
