The coach of the current age group the player is part of must complete a written assessment as to the players ability and suitability to play up an age-group.

The coach of the older age team must also complete a written assessment of the younger person’s ability and suitability to play up an age-group.

The Club Coaching Co-ordinator must indicate in writing his/her agreement that they are happy for the player to play up and are satisfied that the necessary coaching assessments have been carried out.

There must be written parental consent that they agree to the younger person ‘playing up’.

The Club Safeguarding Officer must be informed and copied in to all documents and will certify this request complies with Regulation 15. CSO to sign.

This form MUST be signed off BEFORE the player plays up. Please do not allow the player to play up until signed off paperwork has been sent back to the club (copied to Club Chair of Age Grade and CSO) as this will invalidate insurance and create a safeguarding issue.
